The following errors were introduced as follows.

On p. 314, column 2, line 23: “…region for the 1D case. The…” should be “…region. For the 1D case, the…”; on p. 315, column 1, line 35, Eq. (3b) should be Eqs. (7a) and (7b); on p. 316, column 1, line 11, Eq. (6) should be Eqs. (1) and (2); and on p. 318, column 2, line 3 under Table 7, Eq. (4) should be Eq. (2).
